Your contact details, property and vehicle information, typed or drawn signature, submission date and time, confirmations and optional attachments are sent through FormSubmit to hooked416@gmail.com. Hooked416 uses them to review your authority and coordinate the request. They are not published on this website. Include only information relevant to the requested removal. Contact 416-301-0820 about access, correction or deletion of your information. FormSubmit processes the submission for email delivery and spam protection.
Driver applications and résumés
The driver application form sends your ten answers, PDF résumé, completed Intact Driver Profile Survey, driver’s abstract, licence photo and tow driver certificate (if applicable) through FormSubmit, a third-party form service, to hooked416@gmail.com. FormSubmit processes the submission and the submitted attachments for email delivery, and may use CAPTCHA for spam protection. See FormSubmit’s privacy policy. Hooked416 uses the information to review your interest in work and contact you about recruitment. Your application is not published on this website.
The Intact survey asks for personal details, including licence number, date of birth, employment and claims history, and signed authorizations. Your licence photo contains identifying information and is collected for driver identification and insurance review. Your driver’s abstract includes licence identification and status, demerit points and driving-record information. Read the survey before completing and signing it. Hooked416 may share the survey, abstract, licence photo and tow driver certificate with its insurance broker or Intact for driver insurance review. Keep those personal details out of the website’s free-text answers and résumé. Use the designated fields for your driver’s licence and, if certified, your tow driver certificate. The certificate includes your name, certificate number and expiry date. Do not attach SIN, banking information or other identity documents. Contact 416-301-0820 to ask about your application or request correction or deletion. Limited technical information is used to control repeated submissions and protect the service. Applications are reviewed by people; the website does not rank candidates or make hiring decisions.
The roadside request form
The request form prepares a text message in your browser. Form entries are not submitted to a Hooked416 web database or saved in browser storage by this site. They remain on the current page until you leave or refresh it. You choose whether to copy the prepared text or open your messaging app and send it to 416-301-0820.
When you send a message or call
Your phone or messaging provider processes the call or text. The details you send are received by Hooked416 and used to respond to your enquiry and coordinate the requested service. Please provide only information needed for that purpose. Do not enter payment card information, identity documents or sensitive claim records in the form.
Location permission
The optional “Use my location” button asks your browser for permission to determine your current location. If allowed, it inserts approximate coordinates and a map link into the location field. It does not continuously track you. You can edit or remove those coordinates before preparing the text. You can also enter an address or intersection manually.
Local weather
The weather card defaults to Richmond Hill. Its separate location button uses your position once to choose a nearby supported GTA city on your device. Your coordinates are not sent to our weather provider or saved by this feature. Only the selected city identifier and normal connection information are sent to Environment and Climate Change Canada. Your selected city and a short-lived weather cache are kept for this browser tab session. You can choose a city manually without location permission.
Third-party links and hosting
Links to Instagram, Google Maps and external review pages take you to third-party services governed by their own privacy terms. This website does not include an embedded social feed or advertising tracker. The hosting provider may process technical request logs and any information needed to provide or secure access to the website.
Service requests and appointments
Using this website does not confirm a tow, a price, an appointment or an insurance benefit. Hooked416 must confirm availability, scope, charges and arrangements with you. Vehicle release and collection are by appointment only.
